To complete the H-E-B Customer Satisfaction Survey and enter into the monthly drawing, an entrant must be eligible and comply with the contest rules such as
- Must be a legal resident of Texas.
- Age must be at least 18 years or over on the date of drawing entry.
- You must not be an employee, representative, and principal or member of their immediate families or household of H-E-B, InMoment or any of their subsidiaries, affiliates, advertising and promotional agencies.
- You must visit a participating H-E-B store and receive a purchase receipt with a certificate code and survey invitation printed on it.

- Visit the official H-E-B Online Survey site mentioned on your receipt invitation or go to www.heb.com/survey
- Select your preferred language to continue the survey.
- Enter the Certificate Code including letters from your H-E-B survey invitation in the specific box on the page. (See the example receipt image posted right-hand side of the page if you have a problem to locate the code on your original H-E-B receipt invitation).
- Click on the “Enter” button to begin the survey
- Complete the survey within the designated selection fields marked within the online sweepstakes form.
Make sure, all areas marked with asterisks must be filled out prior to submitting the survey. If you complete all the steps rightly, you will receive one free entry to the applicable month’s draw.

A Few Important Notes on H-E-B Survey Sweepstakes
- Survey entries will be assigned to the market region based on the H-E-B store you visited.
- And mail-in entries will be by the administrator assigned to the market region in which entrant resides or nearest to it based on the zip code or address submitted in the write-in request.
- Participation is restricted to a maximum of one survey completion or mail-in entry every day per person, unique email address, or phone number.
- 50 random drawing will be held within the 13th day of each month.
- Selected winners will be contacted by telephone or email within 10 days of the drawing and they must sign and return an affidavit of eligibility and publicity release within 14 days of receipt.
- The prize will be sent to the winner within 2-4 weeks of receipt of the declaration.
- The $100 prize H-E-B Gift Card may not be used to purchase alcohol or tobacco.
